It is derived from signal detection theory which categorizes the process of identifying signal over noise. This scoring rule is used when candidates are allowed to select an unspecified number of options. They earn points by endorsing the correct options and they lose points by endorsing the incorrect options. Identifying more correct information results in higher scores. The maximum points for these items is the total number of correct options. The minimum points is zero as no negative scores are allowed.

Many of the transition programs, reviewed by the Transition to Practice Committee, focus on safety (2, 19, 20, 27, 29, 30, 36, 37). Clinical reasoning, also called critical thinking, is another essential part of a transition to practice regulatory model. Like most of these elements, it must be integrated through the program. As the Carnegie study (7) points out, this is where nurses learn to think like a nurse. The Dartmouth program (30) is exemplary as it uses simulation to assist novice nurses in making decisions during common clinical events or events that are uncommon, but life threatening. Transition programs that specifically report integration of clinical reasoning/critical thinking include: 19, 20, 24, 26, 27, 29, 36, 37.

Those number of responses then are able for us to map back to the NCLEX scale. Now, what happens is as you accumulate more and more items, every time you answer an item, it takes all the history of every one of the items that you've chosen. So however many points you got for those and has mapped them back onto the NCLEX scale. So that's how kind of points relate to the scale. And then what happens is behind the scenes, as Shu-chuan was pointing out, there's a threshold. There's a cut point about what's passing.

You can think of it as someone repeatedly adjusting the weights on the weight scale to determine the weight of an object. Starting from 2023, you will be given both dichotomous and polytomous scoring items in NCLEX. Dichotomous items can only be scored as correct or incorrect. You'll get one point for a correct response and zero points for an incorrect response. Polytomous scoring means items can have multiple score points such as one, two, or three. The polytomous scoring allows us to distinguish people who know something from people who know nothing. In addition, you will see clinical judgment unfolding case study and the stand-alone items in NCLEX CAT exam.
