An Individual has requests a name change through the ORBS Nurse Portal Process a Name Change Request within a License Application Process: 1. In ORBS Back Office, search for the individual and open their Profile 2. Open the Demographics Module Notice the Name Change Flag under the name with the date of the request in the image above. 3.
