Airfare/Bus/Rail/Lodging/ Meals/Shuttle/Taxi/Other Enter the dollar amount in the row for each type of expense under the date for that meeting. Enter a description for other expenses that do not have a designated line in the Explanatory Remarks section in the right center of the form. When paying for meals for others, please list the name of each person. Mileage Enter the number of miles traveled using your personal AUTO to attend the business meeting.

Support for ongoing operation of NCSBN products and services or of member board operations will not be considered. 1.3 Support to member boards will only be in the form of products and services or time-limited financial transitional resources. 2.0 Standards/Criteria 2.1 All requests shall be submitted in writing to the CEO and must be submitted by the member board executive officer. 2.2 The request must be related to at least one of NCSBN’s program/service areas: a.
